Usually not. Clicking is normal as long as there is no pain or swelling in the joint. If there is pain or swelling, then it should be examined.

Does knee pain always mean osteoarthritis?

Not at all. Knee pain can come from many things: osteoarthritis, sports injuries, a torn cartilage, and even the muscles and tissues around the knee. Pain does not automatically mean arthritis.

Does exercise wear joints out or protect them?

It depends on the type of exercise. If you overdo it, train too much or use a poor technique, you will certainly increase the pain. Otherwise it does not: exercise is supposed to strengthen the muscles and help the pain go away.
