ACL and Meniscus Tears: How the Treatment Depends on Your Age and Activity

There is no single treatment for a torn ligament or cartilage in the knee. Dr. Amr Elyamany explains how the choice depends on your age, your activity and the kind of tear.

ACL tears

  • Young patient who wants to return to sport: arthroscopic (keyhole) surgery to rebuild the anterior cruciate ligament.
  • Over 60: physiotherapy and strengthening of the front and back thigh muscles and the hip muscles.
  • Partial tear, no feeling of the knee giving way, normal daily life and sport: surgery may not be necessary; keep the muscles strong and warm up well before any sport.

Meniscus tears

  • Grade 3 tear: arthroscopy, either to repair the meniscus or to remove the damaged part if it is shredded.
  • Young patient: repair is preferred over removal, especially when the torn area has a good blood supply.

What decides the plan

Your examination, the MRI scan, how stable the knee feels and the demands you place on it. Two people with the same scan can need different treatment.

Key takeaway: The best treatment is the one matched to you, not to the scan alone.

When to see a doctor

  • A knee that gives way or locks after an injury
  • Persistent swelling after a twisting injury
  • Difficulty returning to sport after an injury that seemed to settle
